Offered by Vancouver Coastal Health. Provides outreach services to connect people who have recently experienced opioid overdose and or are at high risk for opioid overdose, to substance use care and support. Social workers help clients start treatment for opioid use disorder, connect to other health and social services, and provide general harm reduction and overdose prevention education.

Intensive Case Management Team

Offered by Vancouver Coastal Health. Provides community-based team care for individuals with moderate to severe substance use problems, concurrent disorders and or mental health illness, and who have many complex needs related to community living, including housing and income. The program also includes a specialized team who work with marginalized women who are not connected to health services and have multiple barriers accessing services.
